Referrals

TADWA works closely with referrers, service providers and customers to ensure referrals are directed to the right service from the beginning.

To make the referral process simpler, TADWA has two referral forms:

  • Services Referral Form – for all TADWA services except occupational therapy
  • OT Referral Form – for occupational therapy assessments and related OT referral pathways

Please choose the form that best matches the service being requested and complete the online referral form.

Services Referral Form

Use this form to refer to any TADWA service other than occupational therapy, including:

  • Assistive Technology (Eg Goods and Equipment, Alert Devices)
  • Home Modifications
  • Bathroom Modifications
  • Helpdesk for Digital Inclusion and Technology Support
  • Vehicle Modifications
  • Freedom Wheels (Modified Bikes & Trikes)

This form is suitable for referrals where an occupational therapy assessment may already have been completed, and the referral is for access to a TADWA service.

Services Referral Form

OT Referral Form

Use this form to request an occupational therapy assessment for:

  • Support at Home
  • NDIS
  • CHSP
  • Private Funding
  • Complex Home Modifications and Design Consultancy

This form should be used for referrals requiring an occupational therapy assessment and clinical OT input.
